I have worked as a Care Coordinator since November 2021 for Ivel Valley South Primary Care Network.

I enjoy supporting patients to find the best way to manage their health and wellbeing. As a care coordinator, I help patients to manage a sometimes complex healthcare system. By having non-clinical, non-judgemental conversations and listening to what’s happening in the patients world, in their words, and understanding what matters most to them, we can link to the appropriate healthcare, social care and community services.   I have worked in the field of health and wellbeing for the over 15 years, and am passionate about maximizing our potential to have the best health outcomes possible.
